The Affidavit of Civil Status also known as Certificate of Celibacy (CERTIFICAT DE CELIBAT), is an affidavit sworn to by the applicant regarding his/her marital status. This is only available to Cameroonian citizens. You may be required by the Dutch authorities to produce this document to prove your unmarried status when here in the Netherlands, you intend to register for, for example, a residence permit or when you are getting married. You must make an appointment and come in person to the Cameroon Embassy in The Hague and bring: A fully filled out but unsigned Affidavit of Civil Status form, from the Embassy Website. You will be required to sign the form in the presence of the consular officer. You also have to present your valid Cameroon passport; The appropriate notary fee; Dutch Ministry of Foreign Affairs Legalization. You may also be required to have the completed Affidavit of Civil Status legalized.
